Notice: Application for Certified Copy of Vermont Birth or Death Certificte

The Vermont Department of Health is initiating a new process for obtaining copies of birth and death certificates, effective July 1, 2019. All requestors must fill out the attached form, which may be submitted to ANY Town Clerk’s office with the State of Vermont. Please contact the Town Office at (802)226-7292 if you have any questions.
